Circular regarding Golden Cards

  Respected    Dear  Parent

                 I feel  honour  inform you that  as per the  direction of  District  Development  commissioner  Srinagar  through the   office  of  zonal  Medical  officer  Batamaloo vide  his  office  letter  No.ZMO/BAT/ 2022 dated :- 2/6/2022 has  a  requested  to  furnish the  details  of  all the  students  of  this  institution   for  obtaining  Ayushman Bharat Golden  cards, in order to ensure  the safety,  security and  health  insurance  of  each and  every  student  of this  institution.

Your  are  as such requested  to provide  us  the  following  details  through  your  ward  immediately  for  further  course  of  action please .

1. 12 digit  Ration Card  number ( Xerox  copy  of  Front  page of  Ration Card).
2. Aadhar  card of the 
3. Registration Fee :- (Rs 30)
4. Laminated Golden  Card ( Rs 50)

This  may  kindly  be  treated  as  most  urgent, please.
